Release of Liability:
As the legal parent or guardian, I release and hold harmless Bloom Dance Studio, its owners and operators from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of Bloom Dance Studio, its owners and operators or in route to or from any of said premises.
I understand and release Bloom Dance Studio the irrevocable and unrestricted right to use, reproduce, and publish photographs and/or video of my child for editorial, trade, advertising, or any other purpose or manner and medium. I hereby release Bloom Dance Studio and its owner, administrative staff and teachers from any and all claims, actions, and liability relating to its use of said photographs. Any exception/objection must be submitted via email prior to the start of the season so we can proactively prevent photos/videos from being taken of your child.
Bloom Dance Studio holds 32 weeks of scheduled classes with an end of year recital in early May. The cost for a class is an annual price but for the convenience of our families broken down and spread out over nine (9) monthly installments (September-May). The first installment will be charged to your account on September 1, 2019 and the final installment will be charged on May 1, 2020. You will pay the same price each month whether it is a long or short lesson month. Tuition does not change when a week is missed for holidays. Nothing is prorated and/or refunded if classes are missed or cancelled due to inclement weather. A non-refundable $20 registration fee will be charged to your account per student upon enrollment.
Payment and Refunds:
NEW this year! - Bloom Dance Studio requires families to make monthly payments by the use of Automatic payment. Your monthly installment will be charged on the 1st of every month beginning September 1st and ending May 1st. We do not send out monthly statements. All tuition is non-refundable. Monthly tuition is required until the studio has received a written email request of cancellation. If for some reason a card is declined, you will receive an email from the studio notifying them of the incident and any error code associated. You will have until the 10th of the month to pay the balance due. Any balances not paid by the 10th of the month will incur a $10 late fee. Refunds will be considered by Bloom on a case by case basis. Registration and costume fees are non-refundable.
A minimum of four students must be enrolled in a class to offer the weekly class. If class enrollment does not meet the minimum student requirement, the class will be cancelled.
Attendance is expected and will be taken at each scheduled class. Missing class will only slow down your dancer's progress. Our desire is to develop your dancer's skills to their fullest potential - this means working hard to attend all classes and giving your best during class time - including arriving to class on time. Advanced dance students are encouraged to arrive 10-15 minutes prior to class to stretch and warm up as to prevent any unnecessary injuries. Attendance is required during dress rehearsal week. Students who miss classes at the end of the season (March-May) or cannot attend their dress rehearsal will be asked to not participate in the recital to be fair to the other students and teachers.
Dress Code & Policies:
Dress code will be enforced for all classes. Level I Ballet Students and older will be asked to sit out if they are not dressed appropriately for ballet class. Hair needs to be tightly secured from face. At Bloom, we value modesty. We ask our students to wear modest dance attire that covers their stomach. No gum is allowed during dance class. Bloom Dance Studio encourages open communication between teachers, staff members, students and parents. We welcome feedback from our families. At the right time, don’t hesitate to discuss any questions or concerns you have regarding your child’s dance experience. However, to honor the time and responsibilities of others, please do not bring up concern to the teacher before, during or after a class, rehearsal or performance. Student Absence/Make-Up Policy:
If a student misses a class due to illness or conflict, we ask the parent to log into their parent portal to record the absence. Make up classes are available to be completed at the student's level or below, and must be scheduled in advance with the Studio Manager. All missed classes must be made up within 2 weeks of the missed class. There is a limit of 3 make-up classes per semester and there is no carry-over of make ups between the semesters.
Weather Closing Policy:
In the event of inclement weather and/or poor road conditions, classes may be canceled for the safety of our students. When Millard Public Schools are closed due to weather, Bloom Dance Studio will also be closed. If inclement weather falls on a weekend, we will send out an email to let families know how to proceed 60 minutes prior to the start of class. Please call Bloom Dance Studio or visit the website to receive weather updates. As weather related closings are outside of the control of Bloom Dance Studio, we do not credit, refund, or prorate tuition for classes canceled due to inclement weather. Students are welcome to request a make up class for weather-related closings.
To respect the time of our teachers and staff members, Bloom Dance Studio requires that students be picked up promptly following their class. The studio will close 5 minutes after the end of the last class. For safety reasons, we ask our students to wait inside the studio space for their parents or guardians to arrive.
Tuition is non-refundable except in the case of serious illness or injury that has been verified by a physician's note or due to a move out of state.
If a student needs to withdraw early from the season, a Class Withdrawl Form must be completed to close the account and prevent further billing. Bloom Dance Studio requires two weeks notice prior to the effective date of the withdrawal. A student missing class does not affect tuition payment. The account will only be closed with a completed Class Withdrawl Form (please email Hannah@danceatbloom.com for a form) and will go into effect two weeks from the date the form is confirmed as received.
Bloom Dance Studio will finish the 2019-2020 season with an end of the year dance recital. Recital week will take place during early-mid May. There will be no recital fee collected as tuition prices now include this fee. Performance tickets will be sold at $8-$12 per seat based on your seating selection. There must be a $0 balance on your account before recital tickets can be purchased. Participation in the Recital is not mandatory, but is highly recommended. Students spend a significant amount of time February-April learning recital choreography as well as performance etiquette and technique. We encourage students who may struggle with stage fright or other similar fears to participate. Recital is a wonderful opportunity for the dancer to to grow in courage and confidence. If you decide to opt out of recital, you can still attend your weekly dance class but in order to avoid costume charges, you must notify us by November 1st via email that you will not be participating. We do our best to keep the multiple classes of individuals and siblings in mind when planning our Recital shows, but we cannot guarantee that any particular classes or siblings will perform in the same recital show.
A Costume Fee of $85.60 including tax (per student, per class) will be charged to your account. A $40 non-refundable deposit per student, per class will be charged on November 15th. The remaining costume balance will be charged on March 15th. Costume Fees include tax, steaming, alterations, tights and accessories. Dance shoes will be an additional charge - if needed. Costumes ordered after January 1st will be charged an additional $15 per costume. Because costumes cannot be returned, costume payments are non-refundable.
Costumes for Combination Classes:
Combination Classes for ages 5 and up will require TWO costumes per class. Total cost for two costumes in a Combo class is $160.50 including tax. There will be an $80 deposit charge made on November 15th for students in Combination Classes. NOTE: Combo classes for “Sprouts” (4 years old) will only have one dance costume and will only perform one dance for their combination dance class.
